1. A complete description of the incident in question.
2. Names, addresses and phone numbers for all those involved, including any witnesses.
3. The name of the insurance company and policy number of any other parties involved in the claim.

Once the claim is reported to the company, we will be sure to provide you with prompt, efficient service so that your life resumes normalcy as quickly as possible.
